Gastroparesis - pathophysiology, etiopathogenesis and possibilities of endoscopic treatment

Background: Gastroparesis (GP) is a disorder characterized by delayed gastric emptying in the absence of a symptomatic mechanical obstruction of the stomach with cardinal symptoms. Two principal pathophysiological mechanisms are believed to be responsible for development of GP: global gastric hypomotility and pylorospasm. As pylorospasm may play an important role in GP, the efficacy of treatment modalities targeting the pylorus, such as gastric peroral endoscopic pyloromyotomy (G-POEM), is currently being intensively studied, as these methods have the potential to help especially patients with severe symptoms for whom other treatment measures are ineffective. The aims of our studies were: (1) to evaluate clinical efficacy of G-POEM in the treatment of patients with severe GP; (2) to evaluate the changes in objective periprocedural parameters measured by gastric emptying study (GES) and functional planimetry; (3) to evaluate safety of G-POEM and (4) to compare the efficacy of two closure methods used to close the mucosal incision in G-POEM: clips and endoscopic suturing (ES). Reduced order model of the swirling flow

Tato dizertační práce je zaměřená na studium vírového copu. Za tímto účelem byl navržen nový vírový generátor umožňující regulaci intenzity zavíření. Chování vírového copu bylo studováno jak numericky, tak experimentálně. Byly použity numerické simulace zachycující i část energetické kaskády turbulentního proudění, dále byly navrženy metody vizualizace dat založené na objemovém renderování a byla testována různá vírová kritéria. Také bylo studováno několik metod dekompozice proudových polí. Nakonec byly navrženy metody pro extrakci kvaziperiodických módů. S využitím těchto metod byl sestaven redukovaný model pro daný rozsah provozních bodů.
Active control of flow past a cylinder

The diploma thesis is considering active flow control. At first part describes boundary layer separation and Kármán vortices formation. It also explains why it is important to control the flow, and the control is divided into basic categories. Then there are described known active flow control methods. Next part defines default problem with Kármán vortices formed behind a cylinder, analyzes flow and suggests useable flow control principles. In last part of the thesis are tested chosen flow control methods. At first there are verified open-loop methods, then closed-loop methods too.
The legal nature of a trust (legal and comparative analysis)

59 ENGLISH ABSTRACT The purpose of this thesis is an interpretation of English trust law, including historical context and comparative application of the acquired knowledge on the Czech legal institute of "svěřenský fond". A critical assessment of the legal regulations governing "svěřenský fond" is followed by theoretical consideration of its legal nature. The work, except the introduction and conclusion, is divided into four main chapters, with the first chapter dealing with the historical roots of the trust. The reader will become familiar with the Roman fideicomissum, the English legal institute called "use" and its influence on the emergence of the law of equity. She will also learn how use turned into trust and what the economic reasons for such a change were. The second part defines the English trust, describes it and clarifies the three main conditions for its creation under applicable law - the three certainties. Further, it will explain who the settlor, trustee and beneficiary are, along with their rights and obligations. It also focuses on the three main types of trusts. Express trusts, created by an act of the settlor and the resulting and constructive trusts, which are created by decision of the courts. Pancreatic Cancer Diagnosis

Pancreatic cancer is a malignant disease with an unfavorable prognosis. Currently, the diagnosis of pancreatic cancer is based mainly on CT, MR and EUS imaging methods, because a reliable biomarker of pancreatic cancer is not available. It is considered a success in the current clinical practice if patients suitable for surgical resection can be selected, because in this group the overall survival is slightly better than in the group where surgery is not technically possible. In the first part, the presented dissertation focuses on testing of already established imaging diagnostic methods used in common practice, i.e. EUS and EUS FNA, as well as modern examination possibilities, such as contrast endosonography, and the comparison of the accuracy of the methods used. The dominant role of the EUS FNA was confirmed. In the next phase of the work, the methodological procedure of EUS FNA collection and the preprocessing of pancreatic tissue samples was tested with regard to the quantity and quality for epigenetic examination and further testing using the prognostic role of KRAS and miR-21. Sarcopenia as a Predictive Factor for Survival after Transjugular Intrahepatic Portosystemic Shunt insertion (TIPS) Introduction: Prediction in medicine most often represents an estimate of the outcome of a particular disease and is one of the basic tools of a physician. In the field of hepatology, the prognosis of a patient with liver cirrhosis is extraordinarily important. As in other cases, it helps to make a better decision about the right patient management. In cases where liver transplantation is indicated, it helps to place the patient correctly on the waiting list according to the urgency of the disease. Assessing the survival of patients after the transjugular intrahepatic portosystemic shunt (TIPS) using the MELD score (Model for End stage Liver Disease) has given to hepatology the universal and most reliable tool to assess the progression of chronic liver disease, not limited TIPS circumstances. The importance of sarcopenia for patients with liver cirrhosis has been widely discussed in the current decade. This bachelor thesis deals with the particle separation from fluid flow. The aim of this thesis is to get acquainted with the already existing methods of separation, to study their principles and to describe devices designed for particle separation. The thesis is divided into two main units, the first explains the individual methods of separation, the second describes the specific particle separators, their advantages and disadvantages. The final part of the thesis is devoted to the design of another approach to separation.

This bachelory thesis is considering a Kármán vorticies and their suppresion. The theoretical part describes principle of phenomenon origin and concepts connected with this topic. Following chapter deals with Kármán vortex street occurrence and danger. In the end of the first part there is an overview of phenomenon suppresion methods. Experiment applies selected suppresoin methods and proves their effectivness.
Study of vortical structures generated by a swirl generator

This bachelor’s thesis deals with the study of vortical structures generated by a swirl generator. The theoretical part is focused on the description of experimental and numerical studies of the vortex rope phenomenon. This part also includes a brief overview of vortex identification methods. The second part is dedicated to the study of vortical structures captured by CFD simulations. The vortex identification methods are compared on an example of the vortex rope.
