Innovatice Methods in the Diagnosis of Sleep Apnea

With the development of diagnostics and the whole field of somnology in the 1970s, the diagnosis of sleep apnea was defined among sleep disorders. Obstructive sleep apnea is its most common type with the highest prevalence, affecting up to 1/3 of the adult population. The disease is characterized by obstruction of the upper airways during sleep, leading to apneic pauses. The final consequence is the development of a number of comorbidities mainly of the cardiovascular system. Not yet fully clarified pathophysiological mechanisms include oxidative stress, inflammatory processes, endothelial dysfunction and others. Diagnosis is multidisciplinary, involving otorhinolaryngological examination of the upper respiratory tract and sleep monitoring, which is demanding in terms of technical equipment, time and experience. Therapy is predominantly conservative, involving lifestyle measures, weight reduction and continuous positive airway pressure therapy. Surgical therapy consists of anatomical widening or removal of the obstruction at various levels of the upper airway. Radek Kučera Summary The interaction between growth factors and cancer incidence and development has been discussed recently. First works suggesting possible connection between increased production of insulin like growth factors with an increased incidence of tumors came already from early 80s of the last century. The aim of my work was focused on evaluation of possible relation between insulin like growth factors or their binding proteins and tumors from different perspectives, to assess their significance and try to determine what role the IGFs and IGFBPs may play in the current tumor diagnostics. The work itself is divided into two parts, the theoretical and practical ones. In the theoretical part, I summarized the action of IGF in human body and also focused on previous findings on the role of IGF system in cancer diagnoses. In the practical part, divided into three subsections, I focused on investigation of relations in a large set of patients with different cancer diagnoses, in a group of women with breast cancer and, finally, I focused on changes of IGF1 levels during follow-up. Utilisation of New Biomarkers for the Optimalization of Diagnostics and Therapy of Tumors of the Gastrointestinal Tract Introduction: Tumor markers are standard diagnostic tools. They are mainly used to monitor the course of the disease and to check the efficacy of the treatment. It is important to observe dynamics. Changing the level of the biomarker can prevent clinical manifestation and lead to early diagnosis of relapse, which in turn means improving the quality of life, including prolonging survival. Recently, we have encountered a number of diagnostic algorithms that suggest algorithms for estimating the risk of tumor presence or the risk of progression of cancer, using statistical methods. Objectives: The aim of this work is to verify new biomarkers for the diagnosis of gastric cancer and to develop an optimal algorithm for their use. Further, to evaluate the importance of cytokeratin markers - Tissue Polypeptide Antigen (TPA) and Tissue Polypeptide Specific Antigen (TPS) for the diagnosis of metastatic colorectal carcinoma in the liver. Prognostic markers of surgical treatment of malignant lung tumours Summary: Introduction: Lung cancer is the most common malignancy in adults in western countries. The ideal strategy of treatment depends on early diagnosis because the clinical stage strongly impacts the prognosis. Among conventional diagnostic methods, we can analyse immunochemical markers to make the diagnosis more precise. Aim: None of the biomarkers has been evaluated sufficiently specific for one type of tumour. In our work, we tried to find a panel of immunochemical markers, which could be useful as well for the assessment of prognosis as for diagnosis. Especially in situations when taking a biopsy is impossible. The next aim of our study was to evaluate the context between clinical-pathological factors and the level of immunochemical markers and their influence on prognosis. Methods: In our study, we classified 109 patients with lung carcinoma. In the control group were also 109 persons. All 109 patients with NSCLC underwent lung resection at the Department of Surgery of University hospital in Pilsen between 17th April 2018 and 26th August 2020. Application of electrical resistivity tomography technique to detect underground cavities

The electrical resistivity tomography (ERT) method is widely used to detect underground cavities because their air, water or clay infill often has a significant geoelectrical contrast to the surrounding rock. The review part of this study summarizes properties of the method and presents examples of investigation of underground cavities in various geological settings, mainly the crevice-type and karst caves and old mining objects. The case study from the Amatérská cave in the Moravian Karst proved the ability of the technique to detect air and clay filled underground cavities above the depth of 40 m. The resolution of the method is lower at greater depths but some indication of deeply situated Pustožlebská Zazděná and Amatérská caves was captured.
