New diagnostic and therapeutic aspects of inflammatiory cardiomyopathy
Kuchynka, Petr ; Šimek, Stanislav (advisor) ; Krejčí, Jan (referee) ; Gregor, Pavel (referee)
Introduction: Inflammatory cardiomyopathy (DCMi) represents a non-familial form of dilated cardiomyopathy (DCM) and endomyocardial biopsy (EMB) is crucial for its diagnosis. Aims: To assess the prevalence of DCMi in patients with DCM of unclear origin, to evaluate the significance of serological tests for antibodies against infectious cardiotrophic agents and to analyze the effect of specific therapy guided by EMB results. Methods: EMB was performed in 56 subjects (mean age 52 ± 10 years) with DCM of unclear etiology and left ventricular (LV) ejection fraction (EF) < 40% with a history of heart failure less than 1 year. EMB samples were analyzed by immunohistochemistry, polymerase chain reaction (PCR) and electron microscopy. Results: Immunohistochemical examination revealed myocardial inflammation in 26 patients (46%), the PCR method detected genome of microbial agents in 32 patients (57%). Electron microscopy showed the presence of particles of microbial agents in 41 patients (73%). Serological blood tests found no IgM antibody positivity against any of the investigated microbial agents. Targeted antibiotic therapy in patients with evidence of Borrelia burgdorferi (Bb) genome in the EMB led to a reduction in LV size, improvement of LV EF and alleviate symptoms of heart failure. Conclusion: DCMi...
Optimality of function spaces for a weighted integral operator
Krejčí, Jan ; Pick, Luboš (advisor) ; Nekvinda, Aleš (referee)
This thesis studies questions related to the boundedness of the integral op- erator T : f → 1 t wf∗ , where w is a given non-increasing function and f∗ is a non-increasing rearrange- ment of a function f. The main goal is to characterize the optimal range for the operator and a given domain and conversely optimal domain for a given range. These results are then illustrated on particular examples. Lastly, some necessary conditions for the existence of optimal space are given. 1
Novel echocardiographic and magnetic resonance methods in diagnostics of cardiac amyloidosis.
Fikrle, Michal ; Paleček, Tomáš (advisor) ; Hutyra, Martin (referee) ; Krejčí, Jan (referee)
Amyloidosis is a term used for a whole group of diseases caused by deposition of a substance called amyloid into different tissues. Amyloid may be produced by a range of pathologic processes. Heart affliction is typical for only several types of amyloidoses. Heart involvement is then the patient`s prognosis major limiting factor. Diagnosis of heart amyloidosis is difficult especially for nonspecific symptoms and nonspecific findings obtained during common diagnostic procedures. The aim of this thesis was to evaluate usefulness of novel diagnostic methods, namely cardiac magnetic resonance with gadolinium enhancement and a simplified echocardiographic evaluation of left ventricular longitudinal strain, in diagnosing amyloid cardiomyopathy. In our first study we examined 22 patients with light chain amyloidosis by echocardiography and also with cardiac magnetic resonance with late gadolinium enhancement. We compared morphologic and functional parameters acquired by magnetic resonance examination, which is considered a gold standard for morphologic and functional measurements, with values obtained by echocardiographic measurement. Afterwards we evaluated the presence and eventually pattern of late gadolinium enhancement during cardiac magnetic resonance exam. From acquired data we conclude that the...
Open-Source Implementation Of Smart Home Gateway For Z-Wave Protocol
Krejci, Jan
Z-Wave protocol is one of the most popular wireless protocols for home automation or smart home. However, in recent years there have been only few studies on practical measurements of this protocol. There are several manufacturers on the market, implying a vast array of devices, as well as deploying a commercial solution to the home is expensive and mostly focused on a single manufacturer’s portfolio. In this article, we designed and implemented our own Z-Wave home gateway for a smart home system based on open source software. We verified the feasibility of the proposed gate using the prototype and evaluated its performance according to the execution time of the command. We also discussed the limitations of the Z-Wave protocol based on our experience.

Postup dezinfekce a čištění textilních závěsů pečetí

Diagnosis of hypertrophic cardiomyopathy with emphasis on genetic testing
Pavol Tomašov Obor: Fyziologie a patofyziologie člověka Abstract Introduction: Hypertrophic cardiomyopathy (HCM) is the most common monogenic cardiac disease with vast genetic and clinical heterogeneity. Genetic testing of HCM patients is important in determining the etiopathogenesis of the disease and is becoming an integral part of patient care. Aim: The aims of our work included the following implementation of genetic testing in HCM in both basic and clinical research: 1. To identify genetic variants in a novel candidate gene NEBL coding Z-disc protein nebulette in HCM patients. 2. To determine clinical and echocardiographic variables associated with the presence of mutations in the most common sarcomeric genes in the Czech population. 3. To find echocardiographic parameters useful in determining the genotype of relatives before the development of phenotype. Results: In a cohort of 95 patients with HCM we identified a novel genetic variant in a conserved spot of NEBL gene (p.H171R) in one patient. The variant was not found in a control population and was predicted to have a damaging effect on the protein's structure. In a cohort of 48 HCM patients tested for mutations in the most common sarcomeric...
