Ethical problems in perioperative care and their impact on patient safety

The dissertation examines the ethics of staff in perioperative care. The aim was to analyze the errors that occur during surgery, to determine the participants in perioperative care who commit these mistakes, and to analyze the reasons why these errors occur. The theoretical part deals with the concept of security from different perspectives of patient safety in surgery, and the international guidelines that direct it. The situation in the Czech Republic in terms of care for patient safety in surgery is also described. One chapter deals with the autor's internship in operating rooms in Sweden. Using the results of observations, the empirical part of the dissertation analyzes the actual behaviour of staff in perioperative care during surgery against the binding documents that ensure safe perioperative care. It also sets out the reasons that lead to non-compliance of health and safety regulations in perioperative theatres. In conclusion, I suggest steps that would help to improve the situation. Motion programmes to influence body composition and physical fitness of seniors

In this work we focused on the application of physical activity, which aimed to reduce the negative impact of physical deprivation and associated functional and structural changes in the musculoskeletal system. The principal criterion was the diagnosis of the organ systems of the locomotor system, focusing on the muscular system. The subsequent application of appropriate forms of exercise intervention was designed to mitigate the effects of the aging process and functional changes in the body of the senior individual. Finally, we wanted to assess the impact of changes in body composition in its own self- sufficiency and independence of older people and the quality of life of older people in terms of physical and psychosocial aspects. The most significant is the loss of muscle mass, sarcopenia development, increase fat mass with frequent reductions in body weight. The focus of our work and its specific purpose is to clarify the influence of the applied physical interventions. To assess changes in body composition are needed for our work used the method of multi Bioanalysis. Results in body composition in older adults have been evaluated and intra-individual measured data were compared with the general senior population. Neuropsychological aspects of preclinical stages of neurodegenerative diseases

Neuropsychological aspects of preclinical stages of neurodegenerative diseases are an extensively studied topic in neuropsychological research. Neuropsychological assessment can be helpful for the estimation of conversion risk in individual cases. The focus of neuropsychological research shifted from the evaluation of dementia to mild cognitive impairment (MCI) or even to the detection of cognitive change before significant cognitive decline. In the theoretical part is presented a contemporary outline of preclinical stages of neurodegenerative diseases. The construct of MCI is the most studied topic in the prodromal stage of neurodegeneration and this part is dedicated to comprehensive analysis of MCI. The empirical research includes five studies on screening methods of cognitive abilities, memory and verbal fluency tests. We present normative and validity data in older adults and show their detection potential in MCI or preclinical stages of neurodegenerative diseases. Furthermore, we tried to show the detection potential of different memory measures in patients with MCI and estimate the relations between hippocampal atrophy and memory performance. Ethics and Power in Social Health Services

The following text is an ethnographical study of relations in which care is shaped. On the example of care homes with special regime I describe how power is made and how different ideals are enacted in care of people with dementia. Through the analysis of daily activities' micro practice, I introduce two possible ways (modes of ordering) of care for frail elderly with cognitive impairment. The first of the modes is based on an enactment of ideals introduced into the practice by the Social Services Act and related system of quality standards: autonomy, individualisation, activation, and inclusion. It relies on standardisation of procedures, detailed specification of actions and management by objectives. The second mode of ordering, which could be located in the stories from care homes, is founded on an inevitable human dependency and vulnerability, and on general human need of giving and receiving care. In it, relationships and care improvisation here and now play the key role. My ethnographic fieldwork has shown that in everyday practice these two ways of doing good care co-exist - they complement each other, overlap and sometimes clash. The Application of the Equity Principles in the Development of the Health Care System

The thesis regarding the topic of Application of the equity principle during the creation of the health system is focused on the ethical principles, which were reflected in the development of the health system in the Czech Republic. Those are solidarity, justice, equality and the right to health. Solidarity is the oldest of the principles, that has had impact on the creation of the individual types of social states in Europe. Solidarity is examined from the perspective of development of social security in times of illness and poverty. The health system of the Czech Republic is appraised in terms of application of the ethical principles at various stages of organisation of the country from the times of the Austro-Hungarian Empire to present. The health systems of Sweden and France were compared to the health system of the Czech Republic in terms of solidarity, justice, equality and right for health in the practical part of the thesis. The main objective of the thesis was to demonstrate whether our health system respects the ethical principles in accordance to recommendations of the WHO document on the grounds of selected indicators. The impact of working memory training on fluid intelligence in young-old adults

Description and explanation of a natural decline of cognitive functions during ontogenetic aging laid in a century of its history grounds for various experiments trying to find wals to slow down or revoke theis process of decline. One of the cognitive domains that has been gaining ever stronger attention among neuroscientists is working memory, which is interconnected with other functions, especially with attention, executive function and fluid intelligence. So far, there has not been reached a consensus regarding a model of working memory. Nevertheless, there are many efforts to describe it, to explain its role within cognitive processes, and to enhance performance. They resulted in various complex and also specialized interventions that are used also in studies focused on older persons. This study adds to the body of literature on transfer effects of working memory training in young-old persons (age 65-75 years; N=65). It is a randomized controlled study with 2 intervened and 1 passive control groups. The goal was to find about an effect of different numbers of training lessons (10 or 20 in 5 weeks) with a computer-based adaptive visually presented verbal n-back, on latent variables of working memory and fluid intelligence.
