Ondráčková, Petra ; Doležalová, Pavla (advisor) ; Nevoralová, Monika (referee)
Barriers to accessing treatment for women with alcohol dependence Petra Ondráčková, bachelor's thesis Abstract Background: Women's alcoholism has generally greater negative impact on women's health, their family and children and therefore it is less tolerated by society and alcohol dependent women are more condemned than men. Women's drinking is negatively perceived by the lay as well as the specialized public thanks to many stereotypes making dependent women's situation worse. Many women stay anonymous because they are afraid to seek specialized help. During treatment from alcohol dependence it is necessary - especially for women - to work with barriers stopping effective treatment. Objective: Main goal of this research was to find out barriers to accessing treatment for women with alcohol dependence. Partial goal of this thesis was an effort to point out to stigmatization of female population using addictive substances in comparison with the male one, which is definitely one of the main reasons why women less often have courage for stepping out from anonymity and using offered services. Methodology: Questionnaire-based research was conducted among patients undergoing treatment from November 2014 to March 2015 at toxic-rehabilitative ward of Psychiatric Hospital Kroměříž where eighty female clients...
Adolescent attachment behaviour and application for primary prevention
Valová, Kristýna ; Doležalová, Pavla (advisor) ; Nevoralová, Monika (referee)
The thesis is attachment and risk behaviour in adolescents, when these topics are linked and the practical (research) part processed within the research sample. The main idea of the research is the assumption that disturbed attachment in adolescence and period of personality formation as well as terrible twos can cause risky behaviour. Objectives of the research work The aim of this thesis is to select a (representative) sample map and describe attachment and risk behaviour in adolescents. Sample file The research group in this thesis consists of adolescents aged 12 to 16 (17) years. Over all, the surfy was attended by 334 respondents and for the purpose of the thesis, a total of 319 answered questionnaires. Respondents were approached in four primary schools and one secondary school within the city of Prague. Data processing methods The thesis was elected quantitative research. It was a survey, within pre-selected primary schools and one secondary school. The research was performed by two questionnaires - IPPA (Inventory of Parent and Peer Attachment; Greenberg, Armsden, 1987), which deals with attachment and SURPS questionnaire (Substance Use Risk Profile Scale), which deals with risk behaviour. Dates were evaluated and processedising SPSS. Subsequently, all dates are summarized and classified...
Intracellular Concentration of Methotrexate in Erytrocytes and MTHFR Polymorphisms: Possible Association with Methotrexate Efficasy and Toxicity in Patients with Juvenile Idiopathic Arthritis
Tuková, Jana ; Doležalová, Pavla (advisor) ; Slanař, Ondřej (referee) ; Pavelka, Karel (referee)
Objective: To investigate whether methylenetetrahydrofolate reductase (MTHFR) C677T and A1298C polymorphisms and erythrocyte concentration of methotrexate (EMTX) could serve as predictors of methotrexate (MTX) efficacy and toxicity in patients with juvenile idiopathic arthritis (JIA). Methods: Genetic analyses and EMTX and folate assessment were performed in 69 JIA patients treated with MTX and classified as full responders (n=51, disease inactivity) or nonresponders (n=18, less than 30 % improvement in paediatric ACR30 criteria while on 15 mg/m2/week parenteral MTX for at least 3 months). Results: Nonresponders were treated with the higher median MTX dose (17.2 vs 12.6 mg/m2/week, P<0.0001), and accumulated more EMTX (217 nmol/L vs 106 nmol/L, P<0.02) and erythrocyte folates (763 nmol/L vs 592 nmol/L, P=0.052) than responders. Analysis of MTHFR allele and genotype frequencies in relation to response failed to detect association. The frequency of any adverse effect was 29.4 % in responders and 33.3 % in nonresponders (P=0.77). The frequency of 677T allele was elevated in patients with adverse effects (52.4 % vs 20.9%, OR=3.88; 95% CI: 1.8-8.6; p<0.002). The probability of any adverse effect was significantly higher in patients with 677TT when compared to 677CC genotype (OR=55.5; 95-% CI: 2.9-1080;...
Art therapy possibilities in a process of treating drug addiction of a person with posttraumatic stress disorder
Pecuchová, Gabriela ; Doležalová, Pavla (advisor) ; Nevoralová, Monika (referee)
The dissertation's subject matter is focused on a definition of art therapy possibilities in a process of treating drug addiction of a person with posttraumatic stress disorder. The theoretical part aims to define the basic information concerning given dual diagnosis and also to reveal the specifics of mentioned mental disorders' relation. In a following part, the dissertation details individual possibilities of art therapy and its influences on already obtained findings concerning the particular dual diagnosis by synthesizing available scientific information from both areas - namely aiming to understand thoroughly the extent of the disorder and consequently to describe art therapy possibilities by applying bio-psycho-socio- spiritual addiction model on its individual dimensions. The research part brings information about which art therapy methods and procedures are used by art therapists working with drug addicted people with PTSD when revealing themes concerning experienced traumas. It focuses on a problem of the absence of any integrated procedure of working with people with PTSD in a process of drug addiction treatment. Within a selection of research group, a method of intentional selection was used. The research is based on a content analysis of half- structured interview data. In the...
Biomarkers in children with periodic fever syndromes
Król, Petra ; Doležalová, Pavla (advisor) ; Šedivá, Anna (referee) ; Krbková, Lenka (referee)
- 4 - Abstract Introduction: Periodic fever syndromes are clinical entities classified as autoinflammatory diseases. The most of the periodic fever syndromes have genetic predisposition (monogenic periodic fever syndromes). PFAPA (Periodic Fever, Aphtous stomatitis, Pharyngitis a Adenitis) syndrome is an idiopathic disease with unknown aetiology. Results: In our study, we described the largest clinical series of patients with PFAPA syndrome from a single center. The laboratory results have confirmed uncomplicated course of PFAPA syndrome. In our measurements we observed significantly higher levels of serum cytokines (IL-1β and IFN-γ) during episodes of fever in PFAPA patients compared to the control group. Our measurements showed increased numbers of plasma cells in the peripheral blood of PFAPA patients. We have found increased levels of naïve CD4 and CD8 T cells and approximately 2-fold higher proportion of CD8 T cells in tonsils of PFAPA patients. Significant differences were also present at levels of IFN-γ, IL-1β, IL-6 and TNF-α in stimulated supernatants compared to supernatants from unstimulated peripheral blood from patients with PFAPA syndrome. Measurements of bacterial profile showed individual microbial profile in patients. Conclusion: Removal of the tonsillar tissue with the potential...
An art therapy constituent of complex institutional program treating substance-related and addictive disorders: Qualitative analysis
Blochová, Markéta ; Doležalová, Pavla (advisor) ; Štefančíková, Mariana (referee)
Background: Art therapeutic intervention has already been part of complex therapeutic programs in institutional treatment of addictive disorders in the Czech Republic for several decades already. However, the form of art therapy is neither codified, nor is there a universally accepted education system. In 2017, the profession of an art therapist was removed from the list of non-medical professions in the field of health care. Aims: To describe how an art therapeutic program is implemented in selected institutional addiction treatment facilities in terms of formal status, documentation, processes, personnel and team collaboration. Methods: Qualitative analysis focused on the treatment program was applied. Five addiction treatment facilities were selected, and cooperation with nine respondents - employees of these facilities - was established. These respondents were selected both intentionally and by self-selection. Semi-structured interviews with art therapists (five respondents) and their collaborators (four respondents) and available written documents anchoring clinical practice were used to obtain data. Results: Art therapy is clearly defined and distinguished from activity and work therapies in the selected facilities. The art therapeutic program is usually carried out once a week mandatory for...
Trauma and connections with the use of selected addictive substances in adolescents girls in institutional care
Karlová, Markéta ; Doležalová, Pavla (advisor) ; Křížová, Soňa (referee)
Background: For girls placed in facilities, the causes of risky behavior and the extent of substance use are not sufficiently mapped. The phenomenon of trauma was chosen for this work as a possible cause of NL use. The data was not systematically processed or analyzed, it could continue to serve as a guide for improving efficiency in these devices. Objective: To identify the most common area of trauma in girls placed in institutional care, to map the prevalence of use of selected substances in the last 30 days and then compare it with the prevalence of use of girls from the general school population (ESPAD 2015). Furthermore, to find out whether more experienced traumatic events lead to a higher prevalence of substance use. Methods: The data were obtained in the form of quantitative research and implemented using the Early traumatic events record questionnaire and the modified ESPAD 2015 questionnaire. The data was transcribed by IBM SPSS Statistics and analyzed in MS Excel using descriptive statistics tools. There were 121 participating respondents. Results: Physical punishment is the most common type of trauma. Institutional girls use four times more cannabis and three times more tobacco than girls in the general school population. Girls from the general school population consume more alcohol. In...
