Case study of physiotherapy treatment of a patient after Total Knee Replacement

iv Abstract Author: Adam Viktora Supervisor: PhDr. Lenka Žáková, Ph.D. Title of the thesis: Case study of physiotherapy treatment of a patient after Total Knee Replacement Objectives: The goal of this thesis is to gather and understand the theoretical knowledge of the knee joint, osteoarthritis, total knee replacement surgery and its rehabilitation, and then evaluate the therapeutic sessions that were applied. Clinical findings: This study evaluated the condition of a 73-year-old patient after total knee replacement of the right knee. The examination showed a reduced range of motion, reduced mobility of joints, muscle imbalances and differences in conditions of soft tissues. Methods: All methods used for this study were based on the knowledge learned and the literature provided by the Faculty of Physical Education and Sports, Charles University. Results: Following the eight therapeutic sessions, the patient had progressed well overall. Progress was seen in terms of pain, range of motion, muscle imbalances and condition of soft tissues. Conclusion: The therapies applied were effective and valuable in this specific diagnosis of the patient. Title: Sonographic Study of hip joints in Ice Hockey Players Aims: This diploma thesis aims to find out whether the game of ice hockey affects the emergence of structural changes in the hip joint and whether the level of performance or the playing position of the ice hockey player affects these changes. Within the framework of the theoretical and research part of the work, the aim is to map the current findings between the relationship between the long-term ice hockey game and its effect on structural changes of the musculoskeletal system. The aim is to clarify the condition of the hip joints of professional czech ice hockey players together with players of the czech university ice hockey league and to subject this condition to control against a sample from the general population who do recreational sports. Methods: This thesis has the research character of an observational experiment. The theoretical basis of the work was developed based on available current Czech and foreign literary sources in English, German, and Czech. Methods: The theoretical part deals with the anatomy and kinesiology of the hip joint, information about rheumatoid arthritis and osteoarthritis, treatment options for osteoarthritis, hip arthroplasty and subsequent physiotherapy intervention after hip replacement. The special section includes initial kinesiological analysis, individual therapies, outcome kinesiological analysis and evaluation of the effect of physiotherapy intervention. The thesis also includes a comparison of the physiotherapy methods selected and used by me with the literature. Klaudia Robová Supervisor: PhDr. Helena Vomáčková, Ph.D. Title: A comparison of results between a Y-Balance Test and a Star Excursion Balance Test among professional floorball players Objectives: The aim of this thesis is to find a relation between the Y-Balance Test and the Star Excursion Balance Test among professional floorball players and simultaneously to evaluate a level of a dynamic postural stability among professional floorball players and afterwards to evaluate a risk level of a lower limb injury. Methods: This thesis is characterised by a non-experimental observational study. A research sample consisted of a deliberately selected group of professional floorball players n = 60, 30 men and 30 women in the age range from 18 to 33 years (the average age was 21.37 +/- 3.63 years), who have actively played the highest senior competition in the Czech Republic at least for one year. Each proband complied certain criteria to be included into the study. The measurement was taken in the field in sports halls of the tested professional clubs. Title: Effect of therapeutic intervention and etiological basis of subacromial impingement syndrome - literature review Aims: Goal of this thesis is to assemble and describe a well arranged summary about clinical diagnosis known as Subacromial impingement syndrome and to conduct a research of literature available from online databases that would draw a conclusion as to whether its pathogenesis, as described by its authors, is consistent with the newest scientific knowledge and research and whether the current surgical treatment of first choice for this pathology is justified. Methods: Current thesis is composed as a literature review with analytical-descriptive character. Review itself is divided into two parts, each of which is dedicated to a different point of view regarding the diagnosis. Methodology chapter defines tasks, approach and criteria according to which this diploma thesis was conducted. Title: Case Study of Physiotherapy Treatment of a Patient with Diagnosis of Coxartrosis after Hip Endoprosthesis Thesis aim: Theoretical description of the hip joint, coxarthrosis diagnosis, endoprosthesis, and practical description of therapy for the acute phase following hip endoprosthesis. Clinical findings: The patient is 62 years old due to coxarthrosis, she underwent endoprosthesis surgery on her right lower extremity, she has a reduction in ROM at the level of the hip in flexion, extension and abduction, she is unable to fully extend her right knee. Swallen anterior and posterior lateral aspect of the tight, muscle weakness in the right lower limb. Summary: The thesis is divided into two parts. The initial section is theoretical, with a brief summary of hip joint anatomy, biomechanics, and kinesiology, as well as coxarthrosis as a disease and its prevention, hip endoprosthesis surgery clinical views and approaches, and hip endoprosthesis rehabilitation protocol. The effect of 14-3-3 protein on intradomain interaction of ubiquitin ligase Nedd4-2

EN The human ubiquitin ligase Nedd4-2 (NEDD4L) ubiquitinates a wide range of membrane proteins and receptors, playing a key role in maintaining homeostasis. This enzyme is regulated by phosphorylation and subsequent interaction with 14-3-3 proteins, which primarily affects its ability to interact with various substrates. However, very little is known about the molecular basis of this protein-protein interaction. In this work, we focused on biophysical characterization of the role of individual phosphorylation sites and also on mapping the structural changes in the Nedd4- 2 protein induced by 14-3-3 protein binding. Our experiments using analytical ultracentrifugation methods revealed that two phosphorylation sites Ser342 and Ser448 are primarily required for stable binding of Nedd4-2 to 14-3-3 proteins. The crystal structure of the 14-3-3ηΔC:Nedd4-2335-455 T367A complex than revealed the simultaneous binding of both phosphorylated residues to the binding groove of 14-3-3 protein. Subsequent modeling based on small-angle X-ray scattering and chemical cross-linking data combined with mass spectrometry indicated extensive structural changes in the individual domains of the Nedd4-2 protein. Insulin-like growth factors 1 and 2 are protein hormones that share similar structural and functional features with insulin. They are commonly abbreviated as IGF1 and IGF2. They play important roles in prenatal development and throughout lifespan. Stimulation of growth of all cell types and significant metabolic effects also belong among their known functions. Dysregulation of their activity or elevated levels were observed in a number of diseases such as diabetes mellitus type 2, cancer, hyperthyroidism, amyotrophic lateral sclerosis, muscle dystrophy, acromegaly, Laron dwarfism and other disorders. Functions of IGF2 are less studied than functions of the other members of the family and even less is known about precursors of IGF2. To address these issues we started with production of IGF2 analogues with enhanced specificity to IR for better understanding the mechanism of receptor activation by IGF2. We produced site-specific analogues modified at sites important for receptor binding. IGF2 analogue with Phe48His mutation is the strongest IGF2-derived binder for IR-A reported ever and it is able to activate IR-A to about 60% of insulin ability. Title: Physiotherapeutic Approaches in the Treatment of Patellofemoral Pain Syndrome - a Systematic Review Objectives: The objective of this diploma thesis is to evaluate physiotherapeutic procedures currently used in the conservative treatment of patellofemoral pain syndrome and to determine the most effective procedure in reducing pain. Other objectives include a summarisation of the issue of patellofemoral pain syndrome from the perspective of etiology and epidemiology, clinical picture, diagnosis and the possibilities of various therapeutic procedures. Methods: The diploma thesis uses systematic literature review and is analytical-descriptive in nature. The methodology chapter sets out tasks, the thesis process and individual research criteria according to which the thesis was written. Results: Following the research, 10 studies were included in the evaluation based on the established criteria. Anterior cruciate ligament rupture in women floorball extraleague players in Czech and Slovak republic - analysis of influences

Title: Anterior cruciate ligament rupture in women floorball extraleague players in Czech and Slovak republic - analysis of influences Objectives: The aim of the diploma thesis is to analyze the main influences on the anterior cruciate ligament rupture in top-level women floorball players in the Czech and Slovak top leagues, with the main focus on the length of player's active career and average week load, including a comparison of incidences of the injury in both countries. Methods: Data collection was done using an anonymous non-standardized questionnaire in electronic form. The research group included 209 top women floorball players aged 15-35 years. 135 players played in the Czech extraleague and 74 in the Slovak extraleague. All 20 teams registered in the highest women's floorball competitions of both countries during season 2021/2022 took part in the research. Results: The results showed a predominance of non-contact injuries (75 %), repeated anterior cruciate ligament rupture occurred in 42.5 % of cases. The effect of the player's active career length (long-term repetitive tissue microtrauma) on the injury was confirmed.
