Rehabilitation nursing of patients after cerebrovascular accidents.

The Bachelor thesis "Rehabilitation nursing of patients after cerebrovascular accident" are theoretical. It includes and describes rehabilitation nursing and the specifics of patient care after cerebrovascular accident. Rehabilitation nursing has significant influence on the patient's life quality. The main goals of the nursing care of such patients are prevention of the decubitus and muscle contractions, positioning, duly verticalization, conditional and passive breathing exercising, and reinsurance of psychological well-being. Equally important are elements of basal stimulation and kinesthetic mobilization, which support patient's activity. The great effort while processing of the thesis was to map and reinsure all the details not only about rehabilitation nursing, but also about areas which have influence on the emergence and development of the cerebrovascular accident and on the patient's life quality. Occurance of the CVA is increasing with increasing age, therefore the incidence of this disease will increase with a gradual increase in the population of seniors. Prevention of the emergence is very important, it begins with public education, continues with the prehospital organization and follows up on high quality treatment in a healthcare facility. Nowadays, in a busy times, we should take care of our health and react on the signals of the organism. Early diagnosis, immediate treatment and high quality rehabilitation are important for cerebrovascular accident. Part of complex cerebrovascular care is a multidisciplinary team that cooperates and complements each other. The aim of this work is to point out the importance of rehabilitation nursing in the care of patients after the CVA, to describe the cooperation of multidisciplinary team and family. The bachelor thesis will be the basis for a seminar on this subject, which is part of the education in the rehabilitation department.

V diplomové práci jsem rozpracovala problematiku poruch příjmu potravy, v teoretické části jsem se zabývala literární rešerší na téma poruchy příjmu potravy u dětí a mladistvých. Ze shromážděných informací jsem vycházela v mé praktické části. Cílem bylo zmapování vztahu dětí k vlastními tělu a jejich životosprávě. Za tímto účelem jsem zvolila metodu dotazníkového šetření. Při zpracování tématu jsem se přesvědčila, že se jedná o velmi složitý problém. Výsledky bohužel nebyly pozitivní, spousta děvčat na základní škole bere dietu jako součást svého života a nejspíše se svým tělem budou celý život přehnaně zabývat. Tyto dívky zřejmě budou náchylné k onemocnění poruchou příjmu potravy. Informovanost není nej lepší, velká procenta dětí tyto nemoci neznají. Mají možnost se o této nemoci dozvědět v hodinách přírodovědy, ale zřejmě tomu tak pokaždé asi není. Myslím si, že by informace o těchto nemocích měly znát již děti na prvním stupni základní školy, není totiž nic neobvyklé, když touto nemocí onemocní devíti, desítileté dívky. Věřím, že situace ve školách se bude postupně zlepšovat a děti budou o těchto nebezpečných nemocech řádně informovány.
Anaemia focusing on macrocytic anaemia

The content of my thesis are macrocytic anaemia. I deal with overall issues of anaemia because it is a widespread disease. According to MCV, I divided individual anaemia into microcytic, normocytic and macrocytic. Then I focused only on the topic of macrocytic anaemia. Macrocytic anaemia is not a frequently occurring type of anaemia. The main causes of macrocytosis include deficiency of folic acid and vitamin B12. I worked with a group of patients, who had examined blood counts for one month in the hospital České Budějovice a. s. In these patients, I examined the frequency of anaemia, which I further divided by MCV into microcytic, normocytic and macrocytic. Then I focused on the macrocytic anaemias. I worked with an automated hematology analyzer Unicel DxH 800 Beckman Coulter. In 12 % of all patients was detected macrocytic anaemia. According my observing, this disease appears more often with men and mostly in the age of 50 and older. In all cases I observed severity of this illness. Majority of patients was examined either for mild degree, or for moderate degree of anaemia. Severe cases were only rare. After that I traced back, if causes of macrocytosis were examined in cases, when patient suffer from anaemia. I worked with ADVIA analyzer and I found out that only 12 % patients were examined for vitamin B12 deficiency and only 11 % were examined for folic acid deficiency. I was wondering, why these parameters weren't examined. I watched therefore, which department examine these two parameters and if there is a connection between frequency of requests for this examination, together with the value of MCV or severity of anaemia. Based on my results, I realized that in cases of macrocytic anaemia often no search was done and that reduces the effectiveness of treatment.
