The Influence of the Prone Position on Systemic Hemodynamics in Spinal Surgery
Kukrálová, Lucie ; Dostálová, Vlasta (advisor) ; Málek, Jiří (referee) ; Kieslichová, Eva (referee)
1 Summary Hemodynamic monitoring is a fundamental part of perioperative care. This thesis deals with hemodynamic changes due to the prone position under physiological, awake state and under general anesthesia during spondylosurgical procedures and to assess the influence of the patient's body support. The next goal was to evaluate the effect of extended non-invasive hemodynamic monitoring on perioperative circulatory stability, used hemodynamic interventions, and the occurrence of postoperative complications. This work consists of two separate studies. In the study "Hemodynamic changes in prone position - a non-invasive physiological study" 12 healthy volunteers older than 18 years were included. Non-invasive hemodynamic measurement was initiated using ClearSight/EV 1000 system in supine position (S position). Cardiac index (CI), stroke volume index (SVI), stroke volume variation (SVV), systemic vascular resistance index (SVRI) and mean arterial pressure (MAP) were recorded. Following parameters were measured using ultrasound at predefined sites: expiratory area of v. cephalica (sVCe), v. saphena (sVSe), v. jugularis interna (sVJe), expiratory and inspiratory area (sVCIe a sVCIi), and maximum and minimum diameter (dVCImax a dVCImin) of v. cava inferior and index of colapsibility (VCI CI) were calculated....
Non-traditional methods of drug administration in anesthesiology and emergency medicine
Kurzová, Alice ; Málek, Jiří (advisor) ; Bláha, Jan (referee) ; Gabrhelík, Tomáš (referee)
1 SUMMARY Introduction The thesis is focused on the research of efficacy and side effects of non-traditional, i.e. nasal administration of oxytocin and nasal and intramuscular administration of remifentanil. Nasal administration has the advantage of being non-invasive and useful in situations with difficult vein access (young children, extensive burns, terrorist attacks and battlefields). The dissertation has two parts that are closely related: nasal, intramuscular and conjunctival administration of remifentanil, nasal and intramuscular administration of oxytocin. Remifentanil is an ultra-short- acting opioid that is registered for intravenous administration only. Its main advantage over other opioids is its rapid metabolism after intravenous administration and absent accumulation even after prolonged infusion. Remifentanil is the only opioid for which, with the exception of two papers, intranasal administration has not been described yet; data on intramuscular and conjunctival administration are completely lacking. Oxytocin is a hormone that is used therapeutically primarily for its peripheral effects in the uterus and breast during pregnancy. However, other central effects have also been observed, i.e. anxiolytic effect, reduction of social stress levels and increase in empathy. It can be useful in...
Apartment building
Málek, Jiří ; Machová, Petra (referee) ; Vajkay, František (advisor)
This bachelor's thesis designs a new building an apartment building in town Jihlava. Building plot is located in calm part called Horní Kosov at western part of town. Designed objekt is free standing a includes one partial basemant and four upper floors. In upper floors are located 14 housing units and in the underground part are located cellar cubicles, boiler room, air conditioning engine room and cleaning room. In ground floor i salso room for bike shed/carriage house. Building has irregular floor plan and in the highest ground are located terraces. Object lays on foundataions of reinforced concrete. Vertical peripheral support system of underground floor is made from monolithic reinforced concrete and inside vertical support system is from concrete blocks. In floors above ground is vertical peripheral support system made from thermal insulation aerated concrete blocks and inside vertical support system is from lime sand blocks. Horizontal bearing constructions are made from monolithic reinforced concrete. At some places are also dies from reinforced concrete. Roof constructions of building object are flat single skin roof with inverse composition.

Patients discomfort after surgery operation, possibilities of prevention
Schmidt, Matouš ; Málek, Jiří (advisor)
The patients in the postoperative period have a group of symptoms called "postoperative disease". It springs from global reaction of human organism to the surgical intervention, anesthesia and the disease that was the reason for the operation. The somatic symptoms of this "postoperative disease" can be palliated by many different methods. From the anesthesiological view it is mainly a pain and postoperative nausea and vomiting (PONV). For the pain management it is necessary to apply individual admission to treatment and exploitation of every available method to ensure maximal possible decrease of patient pain level. In the case of postoperative nausea and vomiting it is necessary to identify the patients with increased chance of PONV and use every possibility for prevention. This means mainly preoperative exhibition of antiemetics, prokinetics and the anesthesiological technique. This work includes questionnaire research achieved on 71 patients after surgical operation. From this study emergesthe fact that even if the patients evaluate the postoperative treatment as good, here is possibilities for more effective analgetic therapy, mainly using the combination of opioids with non-opioids analgetics. In the case of the postoperative nausea and vomiting it is important enhance of prevention for patients with...
