The effects of mid-urethral tape on symptoms of overactive bladder

Background: Mid-urethral tape implantation is at present a common procedure for stress urinary incontinence. It is greatly successful with a low degree of complications. A large part of patients suffering from stress urinary incontinence has co-existing symptoms of overactive bladder (OAB), which can improve in a certain manner after placing of the mid-urethral tape. Data in literature about risk factors for persistence of OAB symptoms are not unanimous. Objectives: To determine the influence of mid-urethral tape implantation on the different symptoms of OAB, to determine risk factors for persistence of OAB symptoms after mid- urethral tape implantation. Methods: The study included 111 female patients suffering at the same time from stress urinary incontinence and symptoms of OAB. All of them underwent implantation of mid- urethral transobturator tape. Using micturition diary and two specific questionnaires (ICIQ- OAB and ICIQ-OABqol) we compared the intenzity of OAB symptoms before and after tape implantation. To determine risk factors for persistence of OAB symptoms we used univariate logistic regression, followed by multivariate regression analysis for selected factors. Facilitation physiotherapy in the tertiary prevention of multiple sclerosis (focusing on neuroplasticity)

Background: Regular physical activity and physiotherapy positively influence the health and physical condition of people with multiple sclerosis (MS). These activities support neuroplasticity of the brain, which can lead to a slowing down of clinical progression. However, the mechanism of this recovery is still not well documented. Nevertheless, imaging methods are currently widely used to help to create a better understanding of the pathophysiological and consequent reparative processes in MS. Aims: This study aims to demonstrate changes in clinical functions, functional activity and brain connectivity (by functional Magnetic Resonance Imaging - fMRI) in MS patients after two months of physiotherapy. It seeks to clarify the preventive importance of regular physiotherapy, and also to provide further insights into the pathophysiological and neuroplastic processes in MS patients. Methods: This is a prospective randomized experimental study with longitudinal observation. The thesis consists of three parts. Theoretical part is based on literary research and the elementary terms are defined here. The analytical part deals with market analysis and its needs, using SLEPT and SWOT analysis. The design part is then the proposal of business plan itself.

2. Summary Treatment of neurogenic detrusor overactivity using botulinum toxin A Introduction: The treatment of neurogenic detrusor overactivity (NDO) is still a serious medical, social and economic problem. Using onabotulinumtoxinA (onaBTA) is the first choice in patients who are refractory to standard anticholinergic therapy or with the occurrence of serious adverse events from this treatment. The standard way of applying onaBTA is intramural injection into the detrusor of the urinary bladder. Aim: To evaluate the effectiveness of the NDO treatment with submucosal applications of onaBTA with regard to the complex mechanism of the action of onaBTA on the urinary bladder. Methods: A randomized, prospective open-label study was performed. The study population consisted of patients with neurogenic detrusor overactivity due to spinal cord injury. A total of 23 patients, in which urinary incontinence persisted due to NDO despite anticholinergic therapy, were included. After randomization, patients were treated with 300 U onaBTA applied in 30 injection sites, sparing bladder trigone.
