Význam tumor infiltrujících lymfocytů jako prognostických faktorů u pacientů po embolizaci portální žíly (PVE) a po PVE s aplikací autologních kmenových Úvod: V důsledku nízkého zbytkového jaterního objemu (FLRV) je až 75% pacientů s metastázami kolorektálního karcinomu do jater (CLM) primárně inoperabilní. Embolizace portální žíly (PVE) pomáhá navýšit FLRV a tím zvýšit i operabilitu, ale selhává až u 40% pacientů. Využití kmenových buněk (SCs) může být způsob jak podpořit efekt PVE. V současné době jsou intenzivně zkoumány interakce imunity a maligních onemocnění. U léčby CLM s použitím PVE nebyla doposud publikována žádná studie, která by se interakcí imunitního systému a nádoru zabývala. Stejně tak nebyl popsán vztah ABC transportérů a CLM u pacientů po PVE. Cíle: Cílem dizertační práce bylo ověřit efekt PVE a intraportální aplikace SCs na nárůst FLRV a progresi CLM. Dalším cílem bylo stanovit tumor infiltrující lymfocyty (TILs) a transportéry ABCC10 a ABCC11 u pacientů s CLM po provedené PVE a jejich klinické dopady. Metoda: Efekt SCs na růst FLRV po PVE byl zkoumán na souboru 63 pacientů (43 pacientů s PVE a 20 pacientů s PVE a aplikací SCs). Z CT vyšetření byly stanoveny FLRV a objemy CLM. Byl sledován jejich růst, vztah k operabilitě, bezpříznakovému období (DFI) a celkovému přežití (OS). Na souboru... Portal vein embolisation (PVE) helps to increase FLRV and so increase the operability. But PVE fails in almost 40 % of patients. Usage of stem cells (SCs) could be the way how to support the effect of PVE. Currently, there are studies of interactions of the immune system and malignancies. We do not know about papers focused on relations of the immune system and CLM in patients treated by PVE. There were not described interactions of ABC transporters and CLM at patients after PVE was performed too. Aims: the aim of this dissertation was to verify the effect of PVE and intraportal administration of SCs on the growth of FLRV and progression of the CLM. Other aims were to evaluate the tumor infiltrating lymphocytes, ABCC10 and ABCC11 transportes in patients treated by surgery for CLM after PVE and their clinical relevances.
