Molekulární biologie nádorů ledvin v klinickém využití MUDr. Ivan Trávníček Souhrn Cíl: Cílem práce je identifikovat možné tkáňové biomarkery ke zhodnocení efektu sunitinibu v léčbě metastatického světlobuněčného renálního karcinomu. Materiál a metody: Retrospektivně hodnocená studie. Ze 119 pacientů léčených v naší nemocnici cílenou léčbou pro metastatický karcinom ledviny (mRCC) od 2005 do 2012 bylo 45 vhodných k další analýze. Z nich 42 (93,3%) bylo světlobuněčných (CCRCC), jeden papilární (PRCC), jeden chromofóbní (CHRCC) a jeden neklasifikovaný RCC. Ze skupiny mCCRCC bylo 40 léčeno sunitinibem, u 14 pacientů (32,6%) byl sunitinib podán po selhání léčby cytokiny (INF-α a IL-2), u 4 pacientů (10%) byl sunitinib druhou linií po sorafenibu. Při operačním výkonu pro primární tumor byly odebrány vzorky tkáně tumoru a zdravého parenchymu ledviny, následně byly vyšetřeny imunohistochemicky a semikvantitativně byla hodnocena přítomnost sledovaných biomarkerů: mTOR, p53, VEGF, HIF1, HIF2, CAIX. Průměrná míra exprese tkáňových markerů byla hodnocena ve vztahu k odpovědi tumoru na podávanou léčbu, ve skupinách pacientů bez odpovědi na léčbu (progrese) a pacientů s odpovědí na léčbu (stabilní onemocnění či regrese tumoru), hodnocení při první kontrole po zahájení léčby po 3 až 7 měsících. Molecular biology of renal tumours in clinical use MUDr. Ivan Trávníček Summary Aim: To identify tissue biomarker that are predictive of the therapeutic effect of sunitinib in treatment of metastatic clear cell renal cell carcinoma (mCRCC). Material and methods: Our study included 39 patients with mCRCC; these were selected from 119 patients who received sunitinib in our hospital between the years 2005 - 2012 according to inclusion criteria of the study. Patients were stratified into two groups based on their response to sunitinib treatment; non-responders (progression), and responders (stable disease, regression). The effect of treatment was measured by comparing imaging studies performed before the initiation of treatment, with those done between 3rd and 7th months of treatment. Histological samples of tumour tissue and healthy renal parenchyma, acquired during surgery of the primary tumour, were examined with immunohistochemistry to detect tissue biomarkers (mTOR, p53, VEGF, HIF1, HIF2, CAIX). The average levels of biomarker expression in both, tumour tissue, as well as in healthy renal parenchyma were compared between the two groups of patients.
