Lynchův syndrom (LS), dříve nazývaný hereditární nepolypózní kolorektální karcinom (HNPCC), je familiární karcinomový syndrom s autozomálně dominantním vzorem dědičnosti. Jeho podkladem je nejčastěji zárodečná mutace některého z tzv. mismatch repair (MMR) genů, které jsou odpovědné za opravy chyb vznikajících při replikaci DNA. Dysfunkce tohoto opravného systému vede ke vzniku a progresi nádorů, především kolorektálních karcinomů (CRC). Podle literatury tvoří LS 3-5 % ze všech CRC. Mezi extrakolonické nádory asociované s LS patří karcinom endometria, ovária, žaludku, tenkého střeva, pankreatu, hepatobiliárního traktu, horního uroteliálního traktu, tumory mozku a sebaceózní nádory kůže. Typickým znakem tumorů asociovaných s LS, při porovnání s běžnou populací, je vznik v časnějším věku. Malignita je většinou prvním projevem a diagnostika LS je tak důležitá nejen pro pacienta samotného a jeho další management, ale především také pro jeho rodinné příslušníky. Výjimku může tvořit vznik sebaceózních tumorů kůže předcházejících interní malignity u jedné z fenotypických variant LS, tvz. Muir-Torre syndromu. Vhodně zvolenými screeningovými metodami lze předejít vzniku maligních tumorů zachycením jejich premaligních lézí, nebo alespoň docílit záchytu časných stádií maligních nádorů. Pro diagnózu LS je nutné... Lynch syndrome (LS), formerly known as hereditary nonpolyposis colorectal cancer (HNPCC) is a familial cancer syndrome with an autosomal dominant inheritance pattern. Its genetic basis is most commonly a germline mutation in one of the mismatch repair (MMR) genes, that are responsible for correction of errors ocurring during DNA replication. Dysfunction of this repairing system leads to the formation and progression of tumors, especially colorectal cancer (CRC). According to the literature LS represents 3-5 % of all CRC. Additional extracolonic tumors associated with LS include endometrium, ovary, stomach, small bowel, pancreas, hepatobiliary tract, upper uroepithelial tract, brain and cutaneous sebaceous tumors. Early age of onset is a typical feature of LS-associated tumors, in comparison with general population. Malignancy is often the first manifestation, therefore the LS diagnosis is important not only for the individual patient and his next management, but also for his family members. An exception is represented by the formation of cutaneous sebaceous tumors prior to internal malignancy in one of LS phenotypic variant, called Muir-Torre syndrome.
