Bakalářská práce se zaměřuje na proces plánovaného propouštění pacientů z akutní do následné péče za účelem poskytnutí co nejoptimálnější péče, vycházející z potřeb hospitalizovaného člověka. Práce se v první části zabývá jednak historií a jednak i současným stavem propojenosti sociální a zdravotní sféry, ale především profesi zdravotně sociálního pracovníka, který hraje klíčovou roli v propojenosti obou rezortů a mezi jehož výchozí kompetence patří právě plánované propouštění. Druhá část práce se věnuje případové studii pražské nemocnice, která autorce posloužila jako příklad dobré praxe v tématu této problematiky. The first part of the thesis explains the history and present state of connection between social and health areas but mainly deals with the position of medical social worker who connects both areas and who cares about discharge planning.
