Invazivní infekce vyvolané rodem Candida, zvláště pak jeho nejvýznamnějším zástupcem C. albicans, patří mezi vážné komplikace, zejména u imunokompromitovaných pacientů. Případný výskyt rezistence k jednotlivým antimykotikům značně komplikuje terapii invazivních mykóz a zvyšuje morbiditu a mortalitu u těchto pacientů. Cílem této práce bylo zavedení rychlé metody detekce rezistence na kaspofungin prostřednictvím hmotnostní spektrometrie MALDI-TOF. Testování bylo prováděno na referenčním kmenu C. albicans CCM8261 a kmenu C. albicans M30 s prokázanou rezistencí na echinokandiny. K měření bylo použito dvou různých nastavení přístroje a pro následné vyhodnocení získaných spekter byla využita korelační a shluková analýza (dendrogram). V průběhu analýzy bylo zjištěno, že publikované nastavení měření nelze aplikovat, neboť pro každý hmotnostní spektrometr je vždy nutno pracovat s nastavením od výrobce. Pomocí shlukové analýzy se podařilo odlišit citlivý a rezistentní kmen. Kvalita získaných spekter byla relativně nízká, což mohlo být způsobeno použitím nevhodného kultivačního média. Shluková analýza prokázala možnost stanovení citlivosti pomocí hmotnostní spektrometrie, nicméně další pokusy zohledňující poznatky získané v této studii budou nutné pro případné zavedení metody do rutinní praxe. More over the presence of antifungal resistance leads to delay of targeted antifungal therapy and increases morbidity and mortality in this group of patients. Therefore the aim was to introduce a rapid method of caspofungin resistance detection by the mass spectrometry MALDITOF. The tests were performed by the use of reference strain C. albicans CCM8261 and caspofungin resistant strain C. albicans M30. Different settings of mass spectrometer were used for the measuring. The obtained spectra were evaluated by correlation and cluster analysis (dendrogram). By cluster analysis it was possible to differenciate the susceptible and the resistant strain. During the analysis we have found, that mass spectrometer settings are unique for each machine and we cannot use the published data. We did not succeed to determine the similarity by correlation analysis. The quality of obtained spectra was quite poor, probably because of non-suitable culture medium used in the test The cluster analysis confirmed the possibility of resistance detection by mass spectrometry; nevertheless more testing profiting from current experience is needed for introduction of this test in routine.
