Trichorhinofalangeální syndrom (TRPS) je malformační syndrom charakteristický kraniofaciálními a skeletálními abnormalitami s autozomálně dominantní dědičností. Dnes se na klinické i molekulární úrovni rozlišují tři typy - TRPS I, TRPS II a TRPS III, jejichž společným rysem jsou řídké vlasy, hruškovitý tvar nosu, dlouhé a ploché philtrum, úzký horní ret, odstávající uši, malá postava a kuželovité epifýzy článků prstů. Podtypy TRPS I a TRPS III jsou výsledkem mutací v genu TRPS1, který je zamapován do oblasti 8q24. Tento gen leží proximálně od genu EXT1; delece obou těchto genů zároveň vede k třetímu podtypu, TRPS II (Langer-Giedionův syndrom, LGS), jehož pacienti kromě obecných znaků TRPS trpí ještě mnohočetnými exostózami a někdy též mentální retardací. V této diplomové práci byla provedena molekulárně genetická analýza na vzorku 16 pacientů, přičemž 8 probandů vykazovalo fenotyp TRPS a 8 probandů mělo pouze izolované exostózy. Z periferní žilní krve pacientů byla vyizolována DNA, která byla následně použita k vyšetření chromozomové oblasti 8q24 metodou MLPA ("multiplex ligation-dependent probe amplification"). Tato analýza odhalila deleci u 1 pacienta s TRPS a u 1 pacienta s exostózami. Sekvenování kódujících exonů genu TRPS1 u zbývajících 7 pacientů s TRPS odhalilo mutace u dalších 2 z nich.... Trichorhinophalangeal syndrome is a malformation syndrome characterized by craniofacial and skeletal abnormalities and is inherited in an autosomal dominant manner. We distinguish free subtypes on clinical and molecular level - TRPS I, TRPS II, TRPS III. All TRPS patients have sparse hair, a pear-shaped nose, a long flat philtrum, a thin upper lip and protruding ears. Skeletal abnormalities include cone-shaped epiphyses at the phalanges, hip malformations and short stature are present. The subgroups TRPS I and TRPS III are result of the mutated TRPS1 gene, which is maped into the 8q24 region. This gene is situated proximal of the EXT1 gene, both genes are affected in a subgroup of patients with TRPS II. These patients suffer more from multiple (cartilaginous) exostoses and mental retardation. In this work we performed molecular genetic analysis of a sample of 16 patients, 8 probands showed a TRPS phenotype and 8 probands had only isolated exostoses. The peripheral venous blood of patients was used to gain purified DNA, which was subsequently used to investigate the chromosome 8q24 region using MLPA ("multiplex ligation-dependent probe amplification").
