[cze][eng] Cílem diplomové práce bylo zmapovat úroveň zdravotní gramotnosti u pacientů s onemocněním diabetes mellitus 2. typu. Dále popsat kompetence sestry v oblasti zdravotní gramotnosti pacientů s onemocněním diabetes mellitus 2. typu a zmapovat výskyt podiatrických ambulancí v ČR. V teoretické části jsme se zabývali zdravotní gramotností v ošetřovatelství, kde jsme probírali, co znamená zdravotní gramotnost a proč je důležité ji zdokonalovat. Zmínili jsme se o edukaci a roli sestry v ošetřovatelství a jak tato role souvisí se zdravotní gramotností. Poté jsme se zaměřili na onemocnění diabetes mellitus 2. typu, jeho charakteristiku, prevenci, vývoj onemocnění, léčbu a nejčastější komplikace, které toto onemocnění provázejí. S tímto onemocněním úzce souvisí životní styl, proto v posledním úseku teoretické části jsou zmíněny pravidla zdravého životního stylu. Výzkumná část byla rozdělena do schématu výzkumného šetření, které mělo tři fáze. První fázi tvořily rozhovory s všeobecnými sestrami pracujícími v ambulanci praktického lékaře. Druhá fáze se uskutečnila pomocí dvou výzkumných souborů. První výzkumný soubor probíhal se sestrami pracujícími na interních lůžkových odděleních a také se sestrami pracujícími ve specializovaných diabetologických ambulancích. Druhý výzkumný soubor tvořili pacienti s onemocněním diabetes mellitus 2. typu. Třetí fáze byla vytvořena dvěma rozhovory a zúčastněným pozorováním. Výzkumné šetření probíhalo s profesorem Kvapilem a dvěma edukačními sestrami, které se aktivně zabývají edukací pacientů s onemocněním diabetes mellitus a jsou proškoleny firmou Lilly. Z výzkumného šetření vyplynulo, že zvyšování zdravotní gramotnosti závisí z největšího podílu na samotném pacientovi, na jeho zájmu, spolupráci a přístupu ke změně svého životního stylu. Nejvíce aktivními sestrami ve zvyšování zdravotní gramotnosti pacientů byly sestry pracující ve specializovaných diabetologických ambulancích, které se snaží úroveň zdravotní gramotnosti zvyšovat především prostřednictvím edukačních sezení s pacienty. Z cíle týkajícího se zmapování výskytu podiatrických ambulancí v ČR bylo zjištěno, že se podiatrické ambulance vyskytují v každém kraji v ČR. Je velmi důležité zvyšovat zdravotní gramotnost pacientů s tímto onemocněním a tím upevňovat jejich zdraví a snižovat riziko komplikací s ním spojených. Výsledky výzkumného šetření poskytují informace ohledně zvyšování zdravotní gramotnosti v souvislosti s onemocnění diabetes mellitus 2. typu.The aim of this diploma thesis was to assess a level of health literacy of patients with diabetes mellitus type 2, describe the nurse's competences in the field of patients' level of health literacy and map the occurrence of podiatric outpatient clinics in Czech Republic. In theoretical part, we were discussing the health literacy in nursing, what the health literacy means and why it is important to improve it. We mentioned the education and the role of nurse in nursing and how this role is related to health literacy. Then we focused on diabetes mellitus type 2, its characteristics, prevention, development of disease, treatment and the most common complications related to this disease. The lifestyle is closely related to this disease, therefore the rules of a healthy lifestyle are mentioned in the last section of theoretical part. Research part was divided into three areas. The first areas consisted of interviews with general nurses working in general practitioner's outpatient clinics. The second area consisted of two research groups. One group consisted of nurses working in inpatient wards and nurses working in specialized diabetological outpatient clinics. Second group consisted of patients with diabetes mellitus type 2. Third area was created with two interviews and participant observation. The research was conducted in cooperation Professor Kvapil and two nurse educators who are actively engaged in education of patients with diabetes mellitus and they are trained by Lilly Company. The research showed that increasing of health literacy depends by the greatest part on patient him-/herself, his/her interest, cooperation and attitude to change of his/her lifestyle. The most active nurses in the field of increasing health literacy were nurses working in specialized diabetological outpatient clinics, who are trying to increase the health literacy particularly by educational sessions with patients. Mapping the presence of podiatric outpatient clinics showed that these clinics are available in each region of Czech Republic. It is very important to improve the health literacy of patients with this disease thereby strengthening their health and reducing the risk of complications associated with this disease. The results of the research provide information about increasing the health literacy in relation to diabetes mellitus type 2.
