Psoriáza je jedna z nejčastějších kožních nemocí, která postihuje okolo 2-3% evropské populace. Její hlavní klinický projev je tvorba charakteristických červených makulózních lézí na kůži. Klíčové patologické znaky psoriatického plaku jsou infiltrace buňkami zánětu, poruchy proliferace a maturace keratinocytů a angiogeneze. Současné strategie léčby psoriázy se soustředí na navození remise. Jeden z historicky nejstarších, účinných, ačkoliv dodnes pouze empirických léčebných přístupů je Goeckermanova terapie (GT). Ta spočívá v kombinaci topické aplikace uhelného dehtu s následným ozářením UV světlem. Cílem této práce bylo zkoumat vliv GT na různé ukazatele zánětu a angiogeneze a tak pomoci rozšířit a prohloubit poznatky o jejích mechanismech účinku a terapeutických terčích. Pro monitorování zánětu byly vybrány pentraxiny CRP a PTX3, jako ukazatele angiogeneze pak VEGF, bFGF a endoglin (CD105). Nemocní a metodiky: Klinická část práce probíhala na Klinice nemocí kožních a pohlavních FN Hradec Králové. Do souboru pacientů byli zahrnuti jak dospělí, tak dětští psoriatičtí pacienti. Aktivita nemoci a účinnost léčby byla hodnocena vyšetřujícím dermatologem pomocí PASI skóre. Vzorky venózní krve byly odebrány před terapií a po jejím ukončení při propuštění z oddělení. Výsledky: PTX3 a CRP: Cílem první práce bylo... Psoriasis is one of the most common skin diseases affecting about 2-3% of the caucasian population. Its clinical presentation is the formation of red scaled plaques on the skin. The main pathological features of psoriatic plaque comprise of skin inflammation, disturbances of keratinocyte proliferation and maturation and angiogenesis. Although the precise precipitating mechanism of psoriasis has not been discovered yet, the key element in psoriasis initiation is T- lymphocyte population. Nevertheless, other inflammatory cells (e.g. neutrophils, macrophages) as well as activated endothelium play a role in its pathogenesis. Indeed, one of the first histological changes in the affected skin is leukocyte infiltration. The so called trafficking of inflammatory cells to the skin is tightly regulated process guided by cytokines especially chemokines. Angiogenesis is a process of new blood vessel growth from preexisted vessel bed. Vessels in the most of the tissues of adult individual remain in a state of quiescence. The rate of endothelial cell turnover is relatively slow (1-2 years). However in cases of physiological or pathological requirements, a new vessel formation can be initiated by the mechanism called angiogenic switch.
