[cze][eng] Z recentních vědeckých informací týkajících se diagnostiky a léčby akutního trombotického uzávěru koronární tepny po koronární intervenci je známo, že významným patofyziologickým procesem je eroze nebo ruptura nestabilního plátu, endoteliální dysfunkce, obnažení subendoteliálních trombogenních struktur a zánětlivý proces v místě ateromového plátu. Zavedení lékem potažených stentů do klinické praxe se významně snížila četnost restenóz v místě implantovaného stentu (a tím i četnost následných revaskularizací) v porovnání se standardními ocelovými stenty [1,39,48,12,13,47,59]. Po prezentování výsledků některých studií v roce 2006 a 2007, poukazujících na možný zvýšený výskyt pozdních trombóz lékových stentů, vzrostl vědecký zájem o patofyziologii a patologii pozdní trombózy i doby trvání duální protidestičkové léčby po implantaci lékem potažených stentů [18,19,20,23]. Studie BASKET-LATE švýcarských autorů poprvé poukázala na možný vyšší výskyt pozdních trombóz lékových stentů a s nimi spojených kardiovaskulárních příhod, nevyjímaje náhlou kardiální smrt. Zatímco restenóza v místě implantovaného koronárního stentu je relativně benigní proces, akutní trombóza ve stentu se považuje za velmi závažnou komplikaci s mortalitou vyšší 50% [24]. Cílem naší práce bylo posoudit četnost kardiovaskulárních příhod...Pathophysiology and clinical implications of late coronary thrombosis after implantation of drug eluting stents in patients with manifest atherosclerosis Abstract Drug-eluting stents (DES) have markedly reduced restenosis rates compared with bare metal stents (BMS) in controlled randomized trials. Concerns have been raised about a possibly increased incidence of stent thrombosis (ST) after discontinuation of dual antiplatelet treatment compared with BMS. While a restenosis is relatively benign process, this serious complication is an issue because mortality of such event is reaching almost 50%. Among well recognized predictors of late stent thrombosis (delayed endothelisation, renal failure, bifurcation lesions, diabetes, premature antiplatelet therapy discontinuation, slow thrombus disappearance), the stent under expansion and stent deployment technique are also considered to be a contributor for development of late stent thrombosis. There was a paucity of data regarding the IVUS guidance during DES implantation. The aim of this study was to assess the role of IVUS guidance during implantation of DES on longterm outcome in patients with high clinical and angiographic risk profile.
