1 V práci je popsán vzácný nález ekrinního syringofibroadenomu (ESFA) asociovaného s dobře diferencovaným dlaždicobuněčným karcinomem. Pacientem byl 85- letý muž, kterému byl odstraněn 2,5 x 2,5 cm velký, hnědě zbarvený ulcerující nodul se zarudlým krvácejícím povrchem umístěný v metakarpophalangeálním kloubu ukazováku levé ruky. Histopatologicky byly nalezeny oblasti dobře diferencovaného dlaždicobuněčného karcinomu, které se střídaly s oblastmi anastomozujícími pruhy, snopci a sloupce epiteliálních buněk rozšiřujících se z exulcerované epidermis do ztluštělé, edematózní myxoidní na cévy bohaté dermis, typickými pro ESFA. Imunohistochemicky byla prokázána exprese proteinu p16 v nádoru, zatímco benigní části s ESFA p16 expresi nevykazovaly. Exprese proteinu p16 v nádoru je velmi často vyvolána HPV infekcí, proto byla ve vzorku provedena širokospektrální detekce HPV. Z nádoru fixovaného formalínem a zalitého do parafínu byla izolovaná celková DNA při speciálních bezpečnostních opatřeních snižujících riziko kontaminace jinými vzorky na minimum. 4 řezy o velikosti 5 µm byly deparafinizovány xylenem a etanolem. Izolace byla provedena manuálně za použití komerčního kitu podle firemního protokolu. Kvalita izolované DNA a absence PCR inhibitorů byla ověřena amplifikací kontrolních genů pomocí multiplex PCR... 1 We report a case of an eccrine syringofibroadenoma (ESFA) associated with well- differentiated squamous cell carcinoma. The patient was an 85-year-old man, who had a 2.5x2.5-cm, brown-colored ulcerated nodule, with a fragile, flesh-colored bleeding surface located beyond the metacarpophalangeal joint of the second finger of his left hand. Histopathologically, there were areas of a well-differentiated squamous cell carcinoma, alternating with the typical area of ESFA characterized by anastomosing cords, strands, and columns of epithelial cells extending from the crusted epidermis into a thickened, edematous, myxoid vascular-rich dermis. Immunohistochemically, the areas with dysplastic epithelium were positive for p16, whereas the benign ESFA parts tested negative. Human papillomavirus was detected in the lesional tissue by polymerase chain reaction, and the subsequent sequencing analysis demonstrated that the virus was close to human papillomavirus type 107. 2 The authors report a case of basaloid carcinoma involving the anus and rectum of a 57-year- old woman.
