Ačkoli je hypertenze studována již po několik desetiletí, stále však není tento problém vyřešen a dokonce se její incidence zvyšuje. Většina případů sekundární hypertenze souvisí s ledvinami, hlavním orgánem regulujícím objem a iontové složení tělních tekutin. Dojde-li k poškození ledvin, nebo přílišnému příjmu soli, nejsou ledviny schopny plnit svou úlohu a dochází ke zvýšení KT. Právě sůl často přispívá k sekundární hypertenzi. Nadměrný příjem soli zvětšuje objem tělesných tekutin a následně se zvyšuje krevní tlak (KT), a jednak působí na mechanismy regulující KT, hlavně endotelinový (ET) a renin-angiotensinový systém (RAS). Endotelin-1 (ET-1) je hlavním účinným peptidem v ET systému. ET-1 je jeden z nejsilnějších vasokonstriktorů. Působí přes ET receptory typu A a B (ETA a ETB receptory), které patří do rodiny receptorů spřažených s G-proteiny. Receptory se poněkud liší v účincích v závislosti na typu buněk, kde se nachází, například ETB receptory v hladkém svalstvu cév se podílí na vasokonstrikci, zatímco v endoteliálních buňkách cév působí naopak vasodilatačně (stimulují produkci NO). V ledvinách ET systém ovlivňuje exkreci Na+, vody a reguluje i průtok krve ledvinami. ET systém je velice složitý a kromě regulace KT a složení tělních tekutin se podílí i na řízení vývoje organismu, angiogenezi,... Although hypertension is studied for several decades, this problem is still not solved. Moreover, its incidence increases. Most cases of secondary hypertension is associated with the kidneys, the most important organ regulating the volume and ionic composition of body fluids. If the kidneys are damaged, or intake of salt is excessive, they are unable to fulfill their role and blood pressure (BP) increases. Especially salt contributes to secondary hypertension. Excessive salt intake increases the volume of body fluids and subsequently increases BP, and secondary act on the mechanisms regulating BP, mainly the renin-angiotensin system (RAS) and endotelin (ET) system. Endothelin-1 (ET 1) is the main active peptide of the ET system. ET-1 is one of the most potent vasoconstrictors. It acts through ET receptors type A and B (ETA and ETB receptors), which belong to the family of receptors coupled with G-proteins. The effects of ETA and ETB receptors differ depending on the type of cells where they are located. For example, ETB receptors in vascular smooth muscle contribute to vasoconstriction, whereas the same receptors on endothelial cells of blood vessels mediate vasodilation (stimulating the production of NO).
