SOUHRN SDN vzniká na základě komplexu patofyziologických pochodů, které nalézáme u pacientů s pozdními komplikacemi diabetu, významně zvyšuje morbiditu i mortalitu nemocných a výrazně zhoršuje kvalitu života těchto pacientů. Téma dizertační práce, imunologické abnormality a infekční komplikace u pacientů se SDN a terapie SDN odlehčením, bylo dále rozděleno do 3 celků: - abnormality imunitních funkcí ve vztahu k diabetu a jeho komplikacím, zejména SDN - infekční komplikace SDN - terapie SDN odlehčením. Problematika poruch imunitních funkcí ve vztahu k diabetu a SDN byla řešena v 5 studiích, infekční komplikace SDN ve 3 studiích a terapie SDN odlehčením rovněž ve 3 studiích. Abnormality imunitních funkcí ve vztahu k diabetu a jeho komplikacím, zejména SDN První studie se zabývala efektem akutní hyperglykémie a/nebo hyperinzulinémie na funkce PMN buněk u zdravých osob. Cílem studie bylo posoudit účinek krátkodobých akutních metabolických změn na fagocytózu a oxidativní vzplanutí PMN buněk in vivo u zdravých dobrovolníků. Akutní hyperglykémie a/nebo hyperinzulinémie byly navozeny třemi clampovými studiemi- HHC, HEC a HGC s endogenní blokádou sekrece inzulínu somatostatinem. PMN fagocytóza a oxidativní vzplanutí byly stanoveny průtokovou cytometrií za bazálních podmínek a po stimulaci E.coli. Výsledky nalezené... The diabetic foot (DF) develops based on a complex of patophysiological processes which are presented in patients with diabetic complications. DF significantly increases a patient morbidity and mortality and markedly impairs the quality of life of these patients. The theme of the dissertation "immunological abnormalities and infection complications in patients with the DF and therapy of the DF by off-loading" was divided into 3 parts: - abnormalities of immune functions in connection with diabetes and its complications, especially with the DF - infectious complications of the DF - therapy of the DF by off-loading. Problems concerning the impairment of immune functions in connection with diabetes and the DF were resolved in 5 studies, infectious complications of the DF in 3 studies and the therapy of the DF by off-loading similarly in 3 studies. Abnormalities of immune functions in connection with diabetes and its complications, especially with the DF The first study deals with the effect of acute hyperglycemia and/or hyperinsulinemia on function of PMN cells in healthy subjects. The aim of this study was to assess the effect of short lasting acute metabolic changes on phagocytosis and the respiratory burst of PMN cells in healthy subjects in vivo.
