Infekční komplikace jsou po alogenní transplantaci hematopoetických kmenových buněk (HSCT) velmi časté. Mimo bakteriálních a mykotických infekcí, se u těchto pacientů na morbiditě a mortalitě podílí významně také virové infekce. Z nich jsou pak nejčastější infekce DNA viry. Od ledna 2001 do října 2008 jsme vyšetřili vzorky od 217 dětských a 382 dospělých pacientů po alogenní HSCT. Vyšetřili jsme celkem 15 757 vzorků (medián 31 vzorků u dětí, 20 u dospělých). Z nich bylo 14 867 vzorků periferní krve a 890 vzorků dalších biologických materiálů (moč, stolice, likvor apd.). Kvantita HSV, VZV, CMV, HHV-6, HHV-7, EBV, adenovirů skupin A-C, BKV a genu lidského albuminu byla ve vzorcích testována pomocí kvantitativní real-time PCR. V případě CMV, HHV-6, HHV-7 a EBV jsme výsledky normalizovali na 10 000 lidských genomických ekvivalentů. HSV a VZV jsme detekovali pouze v malém počtu vzorků. Hranici virostatické terapie CMV reaktivace 100 normalizovaných kopií jsme detekovali u 55 dětí a 123 dospělých, z nich se přes terapii symptomatická CMV nemoc objevila u 68 pacientů. Pokročilá CMV nemoc samostatně, či v souvislosti s dalšími komplikacemi po HSCT se pak projevila u 20 pacientů, u 17 s fatálními následky. U 4 dětských pacientů jsme také prokázali výskyt ganciklovir rezistentní mutanty CMV, což vedlo k modifikaci... Infectious complications are very common after haematopoietic stem cell transplantation (HSCT). Beside bacterial and mycotic infections, viral infections are important cause of morbidity and mortality of these patients. Among viral infections, the most common are infections caused by DNA viruses. Between January 2001 and October 2008, we have tested samples from 217 paediatric and 382 adult patients after HSCT. In total, we have tested 15 757 samples (median 31 samples for children, 20 for adult). Among them, there was 14 867 samples of peripheral blood and 890 samples of other biological materials (urine, stool, CSF etc.). Quantity of HSV, VZV, CMV, HHV-6, HHV-7, EBV, adenoviruses of group A-C, BKV and albumin gene was tested using quantitative real-time PCR technology. Results of CMV, HHV-6, HHV-7 and EBV were normalized to 10 000 human genome equivalents. HSV and VZV were detected in minute number of samples. Threshold for virostatic therapy was assessed to 100 normalized viral copies for CMV. This level was crossed in 55 children and 123 adults. Despite the virostatic treatment, symptoms of CMV infections developed in 68 patients.
