Bronchopulmonální dysplázie (BPD) přispívá výrazně k morbiditě dětí s porodní hmotností <1000g. Experimentální a klinický výzkum v rámci postgraduálního studia se omezil na dvě intervence, které mohou pozitivně ovlivnit postnatální vývoj silně nezralých plic. V animálním experimentu jsme testovali schopnost kyseliny retinové (RA) omezit rozvoj plicního poškození po sedmi dnech expozice 40% nebo 80% hyperoxii. RA signifikantně omezuje hyperoxickou růstovou retardaci, ke které došlo při expozici 80% hyperoxii. Histopatologické změny plicní tkáně byly zmírněny aplikací kyseliny retinové ve skupinách 40A i 80A. Aplikace RA moderuje pokles exprese genu pro VEGF-A při expozici 80% hyperoxii, tento rozdíl ale není signifikantní. Ve druhém animálním experimentu jsme zaznamenali signifikantní nárůst exprese genu proinflamačního cytokinu TNF- ve skupině vystavené 40% i 80% hyperoxii po dobu 72hod. Mechanická ventilace je jedním z rizikových faktorů vzniku BPD. Klinický management strategie optimálního plicního objemu je obtížnější během ventilačního módu HFOV než během konvenční ventilace. Monitorace dechových objemů by mohla napomáhat v kontinuálním monitorování míry ventilace během HFOV. Prezentujeme data anemometricky měřených exspiračních dechových objemů (VTE) během HFOV, jejich porovnání vzhledem k hodnotě... Experimental and clinical research of dissertation thesis has focused on two interventions which might have positive influence on the postnatal development of the immature lung. In animal experiment, we tested the capability of retinoic acid (RA) to attenuate hyperoxic lung injury after seven days exposure to 40% or 80% hyperoxia. RA significantly attenuates the 80% hyperoxic growth retardation. The histological changes were attenuated in the lungs of 40A and 80A groups. The expression of the VEGF-A gene was not significantly influenced by RA in 80% hyperoxia. In another experiment, we detected significant increase of the expression of the gene of proinflammatory cytokine TNF- after 72 hours exposure both to 40% and 80% hyperoxia. Mechanical ventilation still remains one of the risk factors of BPD. The management of optimal lung volume strategy is more difficult during HFOV than conventional ventilation. Efficacy and safety of HFOV is hampered by a lack of reliable bedside in-line monitoring of the patient.
