MUDr. SKALICKÝ, Tomáš Autor se v dizertační práci zabývá radiofrekvenční ablací jaterních nádorů. Při léčení jaterních metastáz využíváme několika druhů léčby. Na prvním místě s nejlepší prognózou dlouhodobého přežiti je radikální jaterní resekce s mikroskopicky čistým okrajem 5-10mm, tzv. R0 resekce. Tito nemocni mají přežiti 5 let v průměru asi 30 - 40% a 10 leté přežití okolo 20%. Radikální resekci jaterních metastáz, přestože je jako jediná potencionálně kurativní, se však podaří provést pouze u 15-25% nemocných s jaterním postižením. Na druhu místě využíváme, u inoperabilních a neresetovatelných tumorů, metody destrukční, kdy tumor není odstraněn, ale je pomoci tepla, chemické látky či chladu destruován. Jde tedy o metody, které buď mohou metastázy zlikvidovat nebo zmenšit jejich rozsah a tím umožnit jejich následnou radikální resekci, či mohou byt použity jako paliativní řešeni a prodloužit život nemocného. Práce vznikla spojením pětileté klinické praxe ve FN Plzeň a experimentální práce. MUDr. SKALICKÝ, Tomáš Five year period of experimental and clinical experience with radiofrequency ablation of liver tumors is described. RFA considerably extends the survival of patients with non-resectable liver metastases. The method has minimal complications and both mortality and morbidity are low.
