Postižení srdce při hypertenzi je jedním z nejzávažnějších důsledků působení vysokého krevního tlaku. Morfologické a funkční změny srdce nejsou způsobeny jen efektem samotného tlakového přetížení, ale na jejich vzniku se podílejí další, hemodynamické i neurohumorální vlivy. V naší práci jsme se zaměřili na srovnání echokardiograficky detekovatelných morfologických a funkčních změn levé komory v důsledku esenciální hypertenze a hypertenze asociované s primárním hyperaldosteronismem (PHA) jako nejčastější příčinou sekundární, endokrinně podmíněné hypertenze. V první části práce jsme se zkoumali rozdíly v geometrii levé komory u mužů s PHA a esenciální hypertenzí po vyčlenění její nízkoreninové formy, u které jsme rovněž jako u PHA předpokládali efekt zvýšeného plazmatického objemu na remodelaci komory. U mužů s nízkoreninovými formami hypertenze jsme pozorovali větší endsystolický i enddiastolický rozměr levé komory, nižší relativní tloušťku stěn a častější výskyt excentrické hypertrofie ve srovnání s muži s normoreninovou esenciální hypertenzí. Zatímco rozměry dutiny LK pozitivně korelovaly s hladinou aldosteronu, tloušťka stěn levé komory byla asociována zejména s tíží hypertenze vyjádřené průměrnými krevními tlaky a počtem užívaných antihypertenziv. Změny v geometrii levé komory by mohly být... Myocardial damage is one of the most serious consequences of arterial hypertension. Changes in the heart structure and function develop not only due to pressure overload itself, but many other hemodynamic and neurohumoral factors contribute to their formation. Our work has compared echocardiohraphic strucutural anf functional changes of the left ventricle, caused by essential hypertension and hypertension associated with primary aldosteronism (PA) as the most common reason for secondary hypertension. The first part of our work focused on the differences in left ventricle geometry in men with PA and essential hypertension after separating it's low-renin form (where, similarly to PA, the plasma volume expansion was considered to have the dominant effect on left ventricle remodelation). In men with low-renin forms of hypertension including PA, we observed greater both endsystolic and enddiastolic diameter of the left ventricle, lower relative wall thickness and more frequent eccentric type of hypertrophy when compared to essential hypertensives with normal renin levels. Whereas left ventricle cavity diameters were positively correlated to aldosterone levels, wall thicknesses were associated mainly with hypertension severity expressed as an average 24hour blood pressure and number of antihypertensives....
